MedicaSoft’s NXT Platform is a scalable, high-performance, high-availability, health data platform for Health Information Exchange, analytics and reporting, and transactional applications such as clinical and patient portals. The NXT Platform is built from AWS’s HIPAA-compliant services. The Platform’s high-availability architecture is redundant at the server level, across availability zones, and across the eastern and western United States when cross-data-center-replication is enabled. Amazon Web Services meets the highest standards for security, such as Cloud Security Alliance Star Level 2, ISO 27001 for security management best practices, SOC1, SOC2, and SOC3. The NXT Platform is HITRUST certified.

The NXT Platform’s front door parses, cleans, normalizes, transforms, and enriches clinical and claims data received from disparate sources in multiple formats and converts it into the HL7 FHIR data model before inserting it into the platform’s data repository. Our front door has parsed CCDs from AllScripts, Aprima, Athena, Cerner, CPSI, DrChrono, eClinicalWorks, Epic, GE Centricity, Medent, Meditech, and NextGen. Standards supported include X12, HL7 V2, HL7 V3, and FHIR. Any FHIR enabled EMR can read and write data via the Platform’s FHIR API. Common Master Patient Indexes such as Initiate and Verato are supported.

High performance repository for clinical and claims data

The NXT Platform’s high-performance data repository uses the HL7 FHIR data model to store clinical and claims data in JSON – a lightweight human readable format. Clinical and claims data in the repository is accessible using an Application Programming Interface (API) that conforms to the FHIR standard. Access to the FHIR API is included with the NXT Platform service. The data repository uses the same document database as leading web companies like LinkedIn and eBay, and includes a Structured Query Language (SQL) that is easy to use. Data at rest in the repository is protected by advanced encryption (AES).

The NXT Platform’s data warehouse combines the extreme scalability of a service hosted at Amazon Web Services with the flexibility and ease of use of SQL. The data warehouse is based on Amazon Redshift – the world’s fastest cloud data warehouse. It has multi-petabyte capacity and integrates easily with a variety of popular analytics tools, such as Tableau, QuickSight, and Excel. MedicaSoft has experience implementing a variety of data models for our data warehouse customers.

MedicaSoft’s commitment to interoperability is evident throughout the NXT Platform. The Platform’s ingest engine takes in clinical and claims data in formats like HL7 V2, HL7 V3, and X12, and then maps, validates, enriches, and transforms that data into the FHIR data model before it is inserted into the Platform’s data repository. Data in the repository is accessible through a FHIR API.

The Platform’s data warehouse is built with the same commitment to open standards. The data warehouse uses standard SQL with features to manage large datasets and support high-performance analytics and reporting. Connections to the data warehouse from analytics and reporting tools use common drivers such as Open Database Connectivity (ODBC) and Java Database Connectivity (JDBC) drivers. 

FHIR Support for Document-Based Exchange

MedicaSoft’s NXT Platform includes FHIR to XDS.b translation that enables access to the contents of an XDS document repository via a call to our FHIR API.

CMS Interoperability and Patient Access

To comply with the CMS 9115 F Interoperability and Patient Access Rule and its related policies, payers must implement the FHIR 4.0.1 standard in accordance with implementation specs that include STU 3.1.0, SMART Core, and Flat FHIR v1.0.0 STU 1 before July 1, 2021.

Meeting the CMS 9115 Interop rule requires more than installing a FHIR server on your network. Additional considerations include:

  • Authentication and Authorization for applications that call the FHIR API 

  • Identity Verification for end users of the applications that call the FHIR API

  • Periodic updates of provider data to keep the Provider Directory current
